Job Summary
We are seeking a registered nurse to join our team of healthcare professionals who are dedicated to delivering exceptional patient care. As a Home Health Registered Nurse, you will provide one-on-one patient-centered care in the comfort of patients' homes.
Responsibilities
- Provide comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Educate patients and families on disease management and self-care techniques
- Collaborate with healthcare teams to coordinate care and services
Requirements
To succeed in this role, you will need:
- A graduate degree in nursing from an accredited institution
- One year of experience in a medical-surgical or related setting
- Current New York State licensure as a Registered Nurse
- Valid NY State Driver's License
