Job Summary
The Licensed Practical Nurse provides general nursing care to patients, families, and caregivers in the Medical Group practices.
They assist and advocate for the patient's health in support of a patient-centered approach and delivery model of care, serving as an active member of the care team.
Collaboration with physicians/providers, registered professional nurses, and multidisciplinary care team members is essential.
Nursing duties are performed within the framework of case finding, health coaching, and supportive care under the direction of a registered professional nurse or licensed physician/healthcare provider.
The Licensed Practical Nurse adheres to the standards outlined for Licensed Practical Nurses in the NYS Education Law, NYS Office of the Professions, and the State Nurse Practice Act.
Core Job Responsibilities
- Gather patient measurements and collect relevant data.
- Record and report patient findings accurately.
- Prepare patients for examinations and treatments.
- Assist with examinations and treatments as needed.
- Perform tasks within the framework of supportive care under clinical direction from licensed healthcare providers or registered professional nurses.
- Support self-care, self-management, and established plans of care.
- Maintain standard precautions at all times.
- Maintain equipment, supplies, inventory, and a safe work environment.
- Stay current with technical knowledge through educational workshops, mandatory in-services, reviewing publications, and participating in departmental committee meetings or workgroups.
- Promote quality improvement initiatives through continuing education, system-wide, and department-specific teams.
- Foster cooperative relationships among the healthcare team by effective communication.
- Respond to patient requests and communicate effectively with licensed healthcare providers who can fulfill these requests.
Education/Experience Requirements
Required:
- Graduate of an accredited school of licensed practical nursing.
Preferred:
- Bachelor's or Master's degree in Nursing from an accredited nursing program.
Licensure/Certification Requirements
Required:
- Current New York State licensure as a Licensed Practical Nurse.
- Basic Life Support Certification; obtain BLS certification within six weeks of hire date.
