About the Role
We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at the Mohawk Valley Health System. As a home health care specialist, the LPN will be responsible for providing direct care and support to patients in their homes under the guidance of a Registered Nurse.
Responsibilities
- Assist in the provision of nursing care to home health patients, collaborating with primary case managers and other healthcare team members.
- Gather data and report significant changes in patient condition to Registered Nurses or Supervisors as needed.
- Instruct patients and caregivers according to the plan of care, implement orders for medications and treatments as prescribed by physicians.
The successful candidate will have a strong commitment to delivering exceptional patient care and possess excellent communication and interpersonal skills.
Requirements
- License and current registration to practice as a Licensed Practical Nurse in New York State.
- Current BLS certification.
- At least two years of experience working as a Licensed Practical Nurse, preferably in an acute care setting.
- Excellent written, verbal, and interpersonal communication skills.
- Proficient with EHR tools.
