Job Description
This Family Medicine Nurse Practitioner role involves providing general medical care and care coordination to various patient levels – adults, women's health, pediatric, and geriatric. You will perform comprehensive evaluations, address both chronic and acute primary care complaints, and establish and document reasonable medical diagnoses.
- Perform face-to-face in-person visits in various settings, including home, skilled nursing facilities, and public locations.
- Schedule patient appointments for visits when appropriate and provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.
