Job Title: Care Connections Nurse Practitioner
Job Summary
The Molina Healthcare Care Connections Nurse Practitioners focus on delivering primary care services in the home, community, and nursing facility settings. We provide needed care in the environment where patients feel most comfortable and receptive, including their homes, nursing facilities, and "pop-up" clinics.
We are seeking a skilled and compassionate Nurse Practitioner to work primarily in non-clinical settings and provide medical care to all levels of patients. Our programs may focus on specific populations, such as adult and geriatric, pediatric, and women's health.
Key Responsibilities:
- Perform comprehensive medical assessments, order appropriate tests and procedures for diagnostic purposes, formulate treatment plans, obtain specialists' consultations as needed, and maintain accurate documentation.
- Provide general medical care and care coordination to various patient levels, including adults, women's health, pediatric, and geriatric populations.
- Address chronic and acute primary care complaints, ascertain medical urgency, and establish reasonable medical diagnoses.
- Create and implement a medical plan of care, schedule patient appointments, and provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.
- Drive up to 120 miles a day to various locations within the assigned region and participate in regional meetings.
Requirements:
- A Master's degree in Family Health from an accredited nursing program.
- An active and unrestricted national certification from a recognized organization, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- A current state-issued license to practice as a Family Nurse Practitioner.
- A current Basic Life Support for Healthcare Professional certification.
- A valid unrestricted driver's license.
Preferred Qualifications:
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, preferably in a home health, community health, or public health setting.
- Previous experience in home health, particularly in managing chronic conditions, and working with underserved populations facing socioeconomic barriers to healthcare.
- Fluency in a language in addition to English is a plus.
