Responsibilities Include but are not limited to:
- Oversee SSD services including care plans and ancillary services for the facility
- Charting and notes as required
- Plan, schedule and implement a program of individual and group activities based on residents’ schedule.
- Assess and complete the sections of the Minimum Data Set (MDS) assigned completely, timely and accurately.
- Gather data and document required ancillary assessments including but not limited to resident assessment protocol.
- Participate as an active member of the interdisciplinary team and attend all resident care conferences.
- Plan and conduct Residents’ Council meetings.
- Plan and implement evening and weekend functions according to facility policy and procedures.
- Take part in in-service education programs and attend all in-service session relating to Activity Program.
- Recruit, schedule and supervise assistants and volunteers.
- Organize and schedule community events related to residents’ interests.
- Supervise barber and beauty parlor activities.
- Maintain an activity attendance record for each resident.
- Observe infection control procedures related to the Activity Program, including identification of residents not appropriate to attend out-of-the-room activity programs for health reasons.
Qualification Include:
- 2 Years as a Social Worker or Social Services Designee
- 2 Years Experience as either a Activity Director or Aid in a Skilled Nursing Facility or Assisted Living
- Computer and Tech Savvy including the use of EMR and Smart Phones and Tablets
- Eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990
- Has 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program
- Is a qualified occupational therapist or occupational therapy assistant;
