Care Coordinator Job Summary:
As a Care Coordinator with Momentum, you will play a critical role in ensuring the highest level of care and support for adults with developmental and intellectual disabilities. In this position, you will collaborate closely with healthcare professionals, family members, and support staff to create comprehensive care plans that meet the unique needs of each individual.

Key Responsibilities:

• Conduct thorough assessments to identify individual needs and goals.
• Develop and implement personalized care plans in collaboration with interdisciplinary teams.
• Coordinate and facilitate communication among healthcare providers, family members, and support staff.
• Provide ongoing support and guidance to individuals and their support networks.
• Monitor and evaluate progress, making adjustments as needed to optimize outcomes.
• Participate in ongoing education and training to stay up-to-date on best practices and emerging trends in care coordination.

Requirements:

• Bachelor's degree in Human Services or a related field.
• Minimum one year of experience in care coordination or a related field.
• Valid driver's license and current proof of insurance.
• Current CPR certification.
• Excellent communication and interpersonal skills.
• Ability to maintain confidentiality and handle sensitive information.
• Physical stamina to perform tasks such as lifting, pushing, and transferring individuals.
