Momentum is hiring a Direct Support Professional II (DSP II) to provide support to adults with disabilities. As a Direct Support Professional II (DSP II), you will be responsible for the implementation of Individual Service Plans (ISP) that will lead to or continue to promote independence and personal skills, well assisting in providing a safe and secure home environment for persons supported in Momentum's Independent and Community Living Services program.

DSP II Essential Responsibilities :
Provides individualized coaching in daily living skillsAssists with daily needs / tasks within persons supported homeProvides education and support in the use of community resourcesMaintains accurate and current records in individuals' communication logs regarding information relevant to Individual Service Plans (ISP)Supports and assists persons supported in following their daily activity schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks, budgeting, ledgers, and transportation, as necessaryPrompts and assists persons supported in medication managementTeaches adaptive living skills and monitors the extent to which they are followedAssists participants in all aspects of personal care, including hygiene and medical / dental appointmentsAssists individuals in attending social activities to further social skillsTrains program participants to access public transportation and assist persons supported in accessing benefits such as Social Security, SSI, IHSS and Section 8 housingAssists persons supported in weight bearing and non-weight bearing transfersDSP II Qualifications :
Must be 18 years of age or olderCurrent and valid California driver license, liability insurance, and reliable means of transportation; requiredHigh school diploma or equivalent; requiredMust have reliable phone / access to phone (monthly stipend)Minimum of one (1) year experience working with people with developmental and / or other disabilities; preferredMust be able to read, write, comprehend, and communicate effectively in EnglishSuccessfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United StatesAbility to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individualsUse of personal vehicle for assignments throughout service areas not normally assigned; highly preferredMust be available to travel throughout Los Angeles and Orange Counties, as needed (mileage reimbursed)Onetime Sign on Bonus will be paid in two payments.
