Human Services Advocate Job Description:
Momentum, a leading nonprofit provider of direct support for individuals with developmental and intellectual disabilities in Southern California, is seeking a dedicated Human Services Advocate to join its team. As a key member of our multidisciplinary team, you will play a vital role in ensuring the well-being and quality of life for adults with disabilities.

Responsibilities:

• Develop and monitor training goals for individuals with disabilities.
• Collaborate closely with healthcare professionals, family members, and support staff to design comprehensive care plans.
• Supervise and oversee the implementation of Individual Service Plans (ISPs) and Behavior Management Programs.
• Provide support and guidance to Direct Support Professionals (DSPs) on delivering high-quality care.
• Communicate effectively with individuals, families, and community resources to ensure seamless transitions and continuity of care.
• Monitor and document progress, making adjustments as needed to optimize outcomes.

Requirements:

• Bachelor's degree in Human Services or a related field.
• Valid driver's license and current proof of insurance.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Ability to maintain confidentiality and handle sensitive information.
• Ability to work independently and as part of a team.
• Physical stamina to perform tasks such as lifting, pushing, and transferring individuals.
