Responsibilities:
- Develop and monitor training goals for individuals with disabilities.
- Collaborate closely with healthcare professionals, family members, and support staff to design comprehensive care plans.
- Supervise and oversee the implementation of Individual Service Plans (ISPs) and Behavior Management Programs.
- Provide support and guidance to Direct Support Professionals (DSPs) on delivering high-quality care.
- Communicate effectively with individuals, families, and community resources to ensure seamless transitions and continuity of care.
- Monitor and document progress, making adjustments as needed to optimize outcomes.
Requirements:
- Bachelor's degree in Human Services or a related field.
- Valid driver's license and current proof of insurance.
- Current CPR certification.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Ability to work independently and as part of a team.
- Physical stamina to perform tasks such as lifting, pushing, and transferring individuals.
