**M-F 8.30am-5pm with some coverage required after hours and on weekends**

Pay range is $108,000-$120,000 per year based on relevant years of experience.

Manage coordination and triage for all Outpatient and Intensive Outpatient referrals to include Adult, Older Adult, and Youth and Family Service Programs. Perform in-person risk assessment of clients being referred and coordinate with referral sources in a manner that ensures the delivery of high-quality, cost-effective care in all service settings consistent with Agency mission, payor, and regulatory requirements. Responsible for handling telephone calls from clients, their legal representatives, case managers, and their significant others, Parole and Probation, insurance companies, health care providers, Social Security, Santa Clara County Behavioral Health Department. Responsible for caseload assignments, assignment of initial intake assessments, arranging initial psychiatric evaluation appointments, and other services that contribute to the achievement of client and work unit goals and objectives. Responsible for overseeing eligibility and payor authorization/re-authorization activities including and/or supervision of administrative staff. Oversee implementation and evaluation of processes to ensure all client data and payor authorizations are correct, verified and entered into appropriate Momentum systems.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Lead and direct development, implementation and management of policies and practices of the intake and assessment department consistent with Agency mission, payor, and regulatory requirements.
    1. Direct adherence to program protocols and staff to ensure there is an assessment and intervention for all referred individuals, including the necessary follow up and use of appropriate consultation;
    2. Direct supervision of Intake Administration Assistants and Clinicians;
    3. Interface with all referral sources including Santa Clara County Behavioral Health Department, and other stakeholders.
  1. Ensure response to all referrals within 24-hour period by telephone or in person.
    1. Arrange and ensure program intake appointments are conducted in a timely manner (within five days after receiving referral).
    2. Participate in fiscal management with CFO, budget development, and payers.
    3. Participate in planning and development of needed program expansion
  2. Arrange initial psychiatric evaluation appointment upon completion of intake.
  3. Direct the day-to-day operations of the Intake and Assessment program for all Outpatient Services. *
    1. Conduct personnel processes, including interviewing, hiring, orientation, training, development, supervision, staff evaluations, and disciplinary action
  4. Assign intake clinicians to meet with clients and clients’ representatives to gather pertinent demographic, eligibility and psychosocial information;
    1. Manage and check the Care Connect referral database 3x day;
    2. Manage and track CSI data information and referrals on a monthly basis;
    3. Manage program protocols: to complete strengths assessments and needs analyses with clients and families;
    4. Make referrals to other public and private social services and community agencies and resources to meet the clients and their families needs.
  5. Maintain timely, accurate, and complete clinical records and other required documentation, and produce clear, accurate reports in accordance with agency, payor, and regulatory requirement, utilizing paper or computer technology.
  6. Coordinate and manage intake functions for outpatient programs by managing mobile staff and multi-site assignment
    1. Provide training, coaching and performance management of program staff to include clinicians and administrative staff.
    2. Conduct evaluations of staff performance, interviewing, hiring, and terminating staff as appropriate.
    3. Provide program team meetings, one on one staff performance associated discussions a minimum of monthly.
  7. Able to travel to sites to support, coordinate and/or conduct intakes to meet timely access requirements.
  8. Demonstrated knowledge of alcoholism/addiction treatment and Motivational Interviewing concepts and techniques.
  9. Able to handle and de-escalate emergency situations.
  10. Flexible schedule: must be willing to workdays and evenings.
  11. Able to write, review and analyze reports that are submitted to the Assistant Chief of Outpatient Programs.
  12. Able to write clear narrative reports and treatment notes.
  13. Assist Deputy Chief of Outpatient Programs in maintaining compliance with CARF, contract, Agency, and regulatory requirements.
  14. Provide transportation in personal or agency vehicle, including but not limited to the transportation of clients and/or program supplies for agency business.
  15. Provide on-call coverage and participate in after-hours rotation, as assigned.

OTHER DUTIES AND RESPONSIBILITIES:

  1.  Maintain effective relations with hospitals, outside case managers, and other community resources.
  2.  Provide transportation in personal or Agency vehicle, including but not limited to the transport of clients, staff, and/or program supplies for Agency business.
  3.  Attend and participate in and/or lead staff, program, agency and community meetings and training, as required.
  4.  Meet continuing education requirements to satisfy regulatory, accreditation, and Agency requirements and ensure continued professional growth, licensure, and knowledge of current trends in mental health and substance abuse.
  5.  Participate in management of team/program discretionary resources available to meet client needs.

KNOWLEDGE, SKILL, AND ABILITIES REQUIRED: (E = Essential; D = Desired) 

  1. Master's degree in the mental health profession and two year’s experience working in human services settings; (E) in a mental health setting and/or with targeted populations, with a minimum of at least one year of experience coordinating referrals and completing intake assessments. (E) (Doctorate education and licensing may be substituted, where appropriate.)
  2. Current licensure from the California Board of Behavioral Sciences (BBS) as a Licensed Marriage, Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC) or Licensed Clinical Social Worker (LCSW), OR current licensure from the California Board of Psychology (BOP) as a Licensed Clinical Psychologist (PSY). (E)
  3. Minimum of one-year experience in management of intake and assessment program, OR supervisory experience in managing an intake and assessment team or program. (D). Minimum of one-year management experience (E).
  4. Meet government and/or regulatory standards for continuing education credits and recertification, including a minimum of 20 hours of directly related professional training, if assigned to a residential facility. (E)
  5. Proven typing speed of 30 net wpm (E); 40 net wpm (D); computer literacy; intermediate and above skill in MS Office applications—Word; Excel; Outlook—and Windows-based databases at a basic level; (E) familiarity with Electronic Medical Records
  6. Demonstrated ability to:
    1. Work independently and in leading a small team environment; (E)
    2. Travel to multi-sites to provide management of intake, assessment and outpatient program function. (E)
    3. Delegate and assign and staff caseloads; manage multiple tasks and demands on a timely basis; (E)

    5. Conceptualize and prioritize the problems of mentally disordered individuals, determining their needs, services, and appropriate treatment methods; (E)
    6. Monitor and support developmental growth and the recovery process; (E)
    7. Effectively conduct and facilitate group rehabilitation services; (E)
    8. Manage maintenance of timely, accurate and complete medical charts and client records and prepare reports; (E)
    9. Employ tact and diplomacy, exercise good judgment, and act calmly in emergency situations; (E)
    10. Work from a strengths-based, harm-reduction, social rehabilitation model; (E)
    11. Work effectively with persons differing in value systems, ethnicity, cultural backgrounds, language capabilities and disabilities; (E)
    12. Work collaboratively with staff and with individuals identified as those with serious mental illness who meet the following criteria: histories of hospitalization, institutionalization, including with the criminal justice system, substance abuse, low engagement in medication treatment, and difficulty in participating in structured activities and living independently; (E)
    13. Supervise staff, trainees, and interns according to BBS and BOP requirements; (D)
    14. Work with populations identified as high risk and/or considered under-represented in the County Mental Health system with multiple barriers; (D)
    15. Exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions. (E)
  7. Demonstrated ability to work with participants who:
    1. May also have histories of assaultive behavior; (D)
    2. May also be developmentally disabled; (D)
    3. May have resided in long term care facilities for extended periods; (D)
    4. May have patterns of service that relied almost exclusively on emergency and institutional care; (D)
    5. May have cognitive difficulties and medical co-morbidities and are considered medically fragile; (D)
    6. May have long-term patterns of homelessness and criminal justice involvement. (D)
  8. Knowledge of:
    1. Clinical techniques for assessment, treatment, and rehabilitation of people with mental illness; problem resolution and conflict resolution techniques; (E)
    2. Community resources, including those required to meet the non-mental health needs of the target population; (E)
    3. Transformational Care Planning (TCP) and its practices.
    4. Mental Health Services Act (MHSA) and Wellness Recovery Action Plan (WRAP). (D)
    5. Substance use treatment services (SUTS) screening and behavioral health integrated services model (D)
  9. Special Requirements: Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Momentum practices.
  10. A valid California Driver License, with access to a reliable vehicle/for driving a company vehicle, current proof of auto liability insurance, and a clean DMV record. (E)
