Job Description
We are seeking a compassionate and dedicated Care Manager (CNA) to join our healthcare team in Southbury, United States. In this full-time role, you will work the evening shift from 2pm to 10pm, providing essential care and support to our patients.
- Assist patients with daily living activities, including bathing, dressing, and grooming
- Monitor and record patients' vital signs, health status, and any changes in condition
- Administer medications as directed by nursing staff
- Provide emotional support and companionship to patients
- Maintain a clean and safe environment for patients
- Collaborate with nurses and other healthcare professionals to ensure comprehensive patient care
- Respond promptly to patient calls and requests
- Document patient care activities and observations accurately
- Participate in team meetings and contribute to care planning discussions
Qualifications
- Valid Certified Nursing Assistant (CNA) certification
- High school diploma or equivalent
- Previous experience as a CNA or in a similar caregiving role
- Strong understanding of basic medical knowledge and patient care skills
- Excellent time management and organizational abilities
- Exceptional communication and interpersonal skills
- Demonstrated problem-solving abilities and attention to detail
- Physical stamina to assist patients with mobility and perform other care-related tasks
- Compassionate and patient-centered approach to caregiving
- Ability to work effectively in a team environment
- Flexibility to work the evening shift from 2pm to 10pm
- Knowledge of elderly care or long-term care practices is preferred
