Job Description

We are seeking a compassionate and dedicated Care Manager (CNA) to join our healthcare team in Southbury, United States. In this full-time role, you will work the evening shift from 2pm to 10pm, providing essential care and support to our patients.

  • Assist patients with daily living activities, including bathing, dressing, and grooming
  • Monitor and record patients' vital signs, health status, and any changes in condition
  • Administer medications as directed by nursing staff
  • Provide emotional support and companionship to patients
  • Maintain a clean and safe environment for patients
  • Collaborate with nurses and other healthcare professionals to ensure comprehensive patient care
  • Respond promptly to patient calls and requests
  • Document patient care activities and observations accurately
  • Participate in team meetings and contribute to care planning discussions

Qualifications

  • Valid Certified Nursing Assistant (CNA) certification
  • High school diploma or equivalent
  • Previous experience as a CNA or in a similar caregiving role
  • Strong understanding of basic medical knowledge and patient care skills
  • Excellent time management and organizational abilities
  • Exceptional communication and interpersonal skills
  • Demonstrated problem-solving abilities and attention to detail
  • Physical stamina to assist patients with mobility and perform other care-related tasks
  • Compassionate and patient-centered approach to caregiving
  • Ability to work effectively in a team environment
  • Flexibility to work the evening shift from 2pm to 10pm
  • Knowledge of elderly care or long-term care practices is preferred
