Primary Care Physician Assistant Role
Job Description:
Monogram Health is seeking a skilled Primary Care Physician Assistant to join our team. In this role, you will provide high-quality, patient-centered care to individuals with chronic kidney and end-stage renal disease.
Responsibilities:
* Conduct comprehensive annual wellness exams and provide personalized medical care to patients
* Educate patients and families on benefits and programs available to improve their quality of life
* Collaborate with primary care providers, specialists, and ancillary services to ensure coordinated care
* Maintain accurate and timely documentation and paperwork
* Ensure quality of care standards as defined by the practice
Requirements:
* Active and unrestricted Physician Assistant license
* Board certified for appropriate licensure (PA-C)
* Current and unrestricted DEA certificate
* Ability to work without direct supervision and practice autonomously
* Access to transportation, valid driver's license, and car insurance
* Proficient with medical instruments and equipment required by the job
* Knowledge of computer-based data management programs and medical records technology
* Excellent communication skills, verbal and written, with patients, family members, physicians, and community representatives
* Understanding of federal and state regulations, including HIPAA and OSHA
