Chronic Disease Management RN
We are seeking a dedicated Chronic Disease Management RN to join our team at Monogram Health. The ideal candidate will have experience working in care management and/or with CKD/ESRD patients.
About the Role:
The Chronic Disease Management RN is responsible for coordinating care and supporting patients with chronic kidney disease and end-stage renal disease.
Key Responsibilities:
- Develop and implement personalized care plans in collaboration with patients' medical providers
- Perform in-home care management visits to execute care plans and monitor patient progress
- Inventory and reconcile medications, coordinate with pharmacists and prescribers, and educate patients on medication adherence
- Deliver individual and group education on CKD, ESRD, dialysis, and associated comorbidities
Requirements:
- 2 years of experience working in care management and/or with CKD/ESRD patients
- Graduate of an accredited School of Nursing and currently licensed as a Registered Nurse in the State of the posted location
- Ability to work independently with minimal supervision and take call remotely on some nights and weekends
