Kidney Health Advocate
Monogram Health is seeking a passionate Kidney Health Advocate to join our team. The ideal candidate will have experience working in care management and/or with CKD/ESRD patients.
About the Role:
The Kidney Health Advocate is responsible for advocating for patients with chronic kidney disease and end-stage renal disease, ensuring they receive the highest quality care and support.
Key Responsibilities:
- Build trusting relationships with patients, their families, and healthcare providers
- Develop and implement personalized care plans in collaboration with patients' medical providers
- Perform in-home care management visits to execute care plans and monitor patient progress
- Provide education and coaching on medications, medical conditions, diet, exercise, and lifestyle choices
Requirements:
- 2 years of experience working in care management and/or with CKD/ESRD patients
- Graduate of an accredited School of Nursing and currently licensed as a Registered Nurse in the State of the posted location
- Ability to show empathy and quickly build relationships with patients and physicians
