Nurse Practitioner - Garland, TX
Job Summary:
The Nurse Practitioner will deliver personalized compassionate medical care to individuals with CKD and/or ESRD/ESKD. The role requires a skilled healthcare professional to work collaboratively with an integrated Care Team, focusing on improving patients' quality of life and slowing the progression of kidney disease.
Key Responsibilities:
- Conduct comprehensive annual wellness exams on patients in their home and virtually.
- Counsel and educate patients and families about benefits and programs to help them live healthier lives.
- Document patient information, including chief complaint, diagnosis, medical history, and treatment plans.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Prescribe medications, order tests, and collaborate with patients' Monogram physicians.
Requirements:
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work autonomously and practice without direct supervision.
- Proficiency in medical instruments and equipment required by the work.
