Clinical Nurse Specialist Opportunity
Job Summary:
We are seeking a Clinical Nurse Specialist to join our team at Monogram Health. As a key member of an integrated Care Team, you will be responsible for providing high-quality, patient-centered care to individuals with chronic kidney and end-stage renal disease.
Responsibilities:
* Develop and implement individualized care plans to address patients' unique needs
* Provide education and support to patients and families on managing chronic conditions and improving quality of life
* Collaborate with primary care providers, specialists, and ancillary services to ensure coordinated care
* Maintain accurate and timely documentation and paperwork
* Ensure quality of care standards as defined by the practice
Requirements:
* Active and unrestricted Registered Nurse and Clinical Nurse Specialist license
* Board certified for appropriate licensure (CNS: ACNPC-AG)
* Current and unrestricted DEA certificate
* Ability to work without direct supervision and practice autonomously
* Access to transportation, valid driver's license, and car insurance
* Proficient with medical instruments and equipment required by the job
* Knowledge of computer-based data management programs and medical records technology
* Excellent communication skills, verbal and written, with patients, family members, physicians, and community representatives
* Understanding of federal and state regulations, including HIPAA and OSHA
