ACT - Nurse

Description

The primary purpose of this position is responsible to be part of the Assertive Community Treatment (ACT) Team which consists of professionals assigned to individuals with a severe, persistent mental illness.  Consumers referred to the ACT program typically have a history of lengthy or frequent hospitalizations and/or a need for intensive community supports provided on an outreach basis.  All members of the team are responsible for all ACT consumers.  The level of intensity of service provided to each consumer by each ACT staff will be a function of the prescribed case management support plan of the physician approved Individual Plan of Service. Services relating to case management support plans will be provided in a direct outreach mode with the aim being to maintain the consumer in the community, reduce hospital recidivism, and reduce inpatient length of stays.  ACT provides 24 hour service and care team members are required to provide scheduled day and night time services as required.  The team as a whole is responsible to the Program Supervisor for delivery of case management services as well as direct and indirect service units consistent with qualitative and quantitative performance goals and standards established for the program.

Qualifications

Must be licensed as a Registered Nurse in Michigan.  Two years’ experience with adults with mental illness preferred.  Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served.  Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group - (adult and geriatric).

Salary : $32 - $40
