What are the responsibilities and job description for the Service Coordinator - CCS/CLTS position at Monroe County, WI?
Do you have a passion for helping others? If so, the Monroe County Dept. of Human Services might have the job for you. We are hiring full-time Service Coordinators. Successful candidates will have a Bachelor’s degree with a major in social work, sociology, psychology, guidance and counseling, or a general college degree with two years professional experience in a Human Services agency. A valid Wisconsin driver’s license is required. Credentialing by the Wisconsin Department of Safety and Professional Services as a certified Social Worker, or candidates eligible for credentialing through another human services related field with a clearly identified path to social work licensure may be considered.
Our Service Coordinators serve participants in the Comprehensive Community Services (CCS) and Children’s Long Term Support (CLTS) programs. Service Coordinators assess and evaluate prospective service recipient’s immediate situation, determines appropriate response, and assures that response is made; they assess and intervene as necessary in crisis situations and refer to and involve other community resources as needed. This position works with the service recipients to assess and develop goal-oriented, time-limited service plans to address identified needs. Service Coordinators work to protect children, elderly, and at-risk adults while striving to maintain existing family units and individual autonomy in the community.
Starting salary for this position is $24.47/hr. for non-credentialed candidates and $25.94/hr. for certified social workers.
