As a Clinical Liaison, you will manage patient referrals, assess eligibility for admission, and ensure a seamless process for discharges and transfers.
Key Responsibilities :
- Patient Referrals & Pre-Screening : Respond to patient referrals by reviewing medical records and insurance information to determine eligibility for admission to the skilled nursing facility.
- Eligibility Determination : Conduct pre-screening assessments to ensure potential residents meet clinical and payer requirements for admission.
- Admission Coordination : Assist patients and families in scheduling initial appointments or treatments once eligibility is confirmed.
- Discharge & Transfer Coordination : Facilitate patient discharges and transfers, ensuring smooth communication and transition of care.
- Promote Services : Actively promote the services of the facility to qualified patients and their families or caregivers, educating them about the benefits of skilled nursing care.
- Collaboration : Work closely with internal teams to help maintain optimal facility occupancy and ensure timely and accurate processing of admissions.
