Description

Summary and Qualifications

This position provides direct care and services to residents.

  • Must maintain current certification/licensure as a certified nursing assistant.
  • Must meet a minimum of 12 hours of nurse aide training annually.
  • Able to safely perform the essential job functions with or without reasonable accommodation.

Essential Functions

  • Provides services in accordance with facility policies and in compliance with state and federal laws and regulations to enhance the quality of life for residents.
  • Recognizes changes in the resident and the resident's functioning and reports the changes to a nurse for follow-up.
  • Assists residents with activities of daily living including eating, bathing, dressing, toileting, transferring and ambulation. Ensures that residents are well groomed.
  • Ensures there is privacy for the resident during times when delivering care.
  • Assists with serving meal trays, setting up and feeding residents as necessary.
  • Cares for clean/soiled linen in compliance with facility policies.
  • Maintains clean and orderly resident rooms on a daily basis.
  • Accurately collects the readings on vital signs and reports any abnormal readings to the Charge Nurse.
  • Effectively measures weight of resident as assigned.
  • Tums or repositions residents who are bedridden.
  • Follows the plan of care and completes assigned duties in a timely manner. Answers resident call lights promptly and ensures that it is within easy reach of the resident.
  • Stores and transports linen according to infection control policies.
  • Timely and accurately documents all pertinent information regarding residents, consistent with facility policy
  • Uses personal protective equipment when at risk of exposure to blood or other potentially hazardous body fluids and all other times indicated by facility policies and procedures.
  • Responds promptly and appropriately to emergency situations.
  • Adheres to and support resident rights.
  • Reports any malfunction or broken equipment to the Charge Nurse. Removes such equipment from service and labels according to facility policy.
  • Respects and adheres to the Ethical and Religious Directives for Catholic Health Care Services.
  • Complete assignments in a timely, complete and accurate manner.
  • Consistently supports the mission and values of the facility.
  • Monitors the workplace for safety and fire hazards reports potential problems.
  • Participates in fire and disaster drills.
  • In the event of an emergency, carries out assigned duties to ensure resident safety.
  • Completes assignments in a timely, complete and accurate manner.
  • Understands and complies with responsibilities concerning resident rights, confidentially, infection control, safety, accident and fire protection.
  • Demonstrates effective time management skills and work practices.
  • Attends mandatory inservices.
  • Adheres to dress code.
  • Reports to work on time and as scheduled.
  • Complies with all facility policies and standards regarding ethical business practices
  • Participates in performance improvement and continuous quality improvement activities.
  • Adheres to the facility's Code of Conduct.
  • Other tasks as assigned.

Physical and Sensory Requirements

  • Ability to read and vyrite English in a legible and understanding manner.
  • Ability to remain clam in stressful situations and to take appropriate actions. Ability to communicate with residents, families, staff, medical personnel, regulatory agencies, and consultants.
  • Ability to walk, reach, bend, lift, grasp, pull, stretch, and stoop; possess fine hand coordination.
  • Ability to transfer residents from different levels and lift residents that are of equal or greater weight than self.
  • Ability to visually assess all aspects of resident conditions and care.
  • Ability to distinguish smells and temperatures and to hear and respond to pages. Ability to instruct nursing personnel concerning training and education.

As appropriate to the position, responds in a timely manner to phone calls and emails.
  • Critical thinking: Able to analyze a situation and make an informed decision. Able to understand problems and come up with solutions.
  • Professional boundaries: Creates a respectful, trusting, safe and secure professional relationships with all residents, family members, and visitors; does not share confidential information about the facility or other employees with residents.
