Job Description:
Under the leadership of the Diabetes Service Program Manager, the Diabetes Program Coordinator is responsible for program development and management that maintains an interdisciplinary approach and collaboration of patient-centered care.
The Coordinator plays a key role in supporting quality initiatives through ensuring compliance with regulatory requirements, collection and analysis of quality and performance improvement data, assisting with development of future performance improvement targets and quality initiatives, and development or revision of department policies and procedures. In collaboration with the Manager, they are responsible for maintaining:
1) National Standards for Diabetes Self-Management Education certification as set forth by the American Diabetes Association’s Education Recognition Program.
2) National Standards for inpatient and outpatient diabetes management as set forth by the American Diabetes Association.
The Coordinator:
  • Maintains oversight of all databases (performance metrics and volume) related to the program and provides reports required or necessary to the appropriate committees.
  • Works in an independent and interdependent relationship with members of the Diabetes Service team and inpatient/outpatient medical staff, which allows for consultation, collaboration, or referral.
  • Establishes and maintains patient support programs and materials and promotes community awareness and education.
  • Analyzes new knowledge gained from conferences, workshops, professional literature, or “hands-on training” and assimilates this knowledge into clinical practice and staff education as appropriate.
  • Maintains registered nurse competencies, skills, and knowledge to competently provide appropriate patient care.
The Coordinator position is a combination of administrative and RN duties as a Certified Diabetes Care and Education Specialist for patient/family visits.
The Coordinator ensures staff have the appropriate orientation/training and competence for the care/service provided and that all staff adhere to Montage Health mission and vision, adhering to values and service standards for professional conduct. This is achieved through supportive training, coaching, developing, evaluating staff performance, policy/procedure enforcement, and corrective action.
Experience:
Must have at least three years of clinical nursing experience. A focus in diabetes education is required. Two years of experience as a diabetes educator working with patient education is required. Formal and informal teaching experience to include facilitating and teaching individuals and groups is required. Two or more years of program coordinator experience preferred.
Education:
Bachelor’s Degree in related field required. Master’s Degree preferred.

Licensure/Certifications:
State of California Board of Registered Nursing licensure as a registered nurse required. American Heart Association Healthcare Provider BLS certification is required. Certified Diabetes Care and Education Specialist (CDCES) credential preferred upon hire and must be completed within 24 months from hire date. Current valid driver’s license and functional transportation are required.

$102.76
The hours employees work determine when a shift differential is paid.
Hourly Evening Shift Differential: $4.81
Hourly Night Shift Differential: $7.21
