Bachelor Level Social Worker/Case Manager - Montcalm Care Network

Competitive pay and benefits with average pay for this position ranges from $51,380 to $73,000 depending on experience.
Case Managers provide community-based outreach to persons with mental illness and are responsible for linking, coordinating, monitoring and psychoeducation. Our case management model utilizes a team approach with nurses, peer supports, and paraprofessional staff providing comprehensive care.
Position will provide integrated care management services to children with serious emotional disturbances, developmental disabilities, and/or autism, and their families.

MCN is an approved National Health Service site (NHSC, a loan repayment program); in addition to Public Service Loan Forgiveness (PSLF), and the MDHHS Behavioral Health Loan Repayment Program. Additionally, MCN offers tuition reimbursement of up to $5,250 per year.
Employment Qualifications:
Education: Possession of a bachelor’s degree in social work, sociology, psychology or a related human services field.
Experience: One year of professional experience in a mental health agency with experience working with the children's population or other mental health population.
Must be a CMHP (Children's Mental Health Professional).

Other Requirements:
Possession of a valid State of Michigan driver's license and ability to obtain a chauffeur’s license within 30 days of hire.
Local travel is an essential function of this role.
