What are the responsibilities and job description for the PM Care Partner in Memory Care position at MonteCedro?

We're looking for a full-time PM shift care partner to work with residents living in memory care at our beautiful senior living community located in Altadea, California.

Pay : $20 Benefits & Shift Differentials Pay

Eligible for Shift Differentials : additional pay premiums to compensate employees for working hours and shifts other than regular day shifts.

Here’s how it works :

3% for evening shifts
5% for NOC shifts
7% for weekend day shifts
10% for weekend evening shift
12% for weekend NOC shift

What You’ll Do :

Promotes independence while assisting residents with activities of daily living including but not limited to personal hygiene, dressing, ambulation, and meal service in accordance with the care plan.

Encourages socialization and physical activity by transporting or escorting residents to meals, activities, appointments, and outings as required.

Responds quickly and courteously to resident calls for emergency and non-emergency assistance.

Observes resident regularly and reports any change in the condition to supervisor and nurse on duty. Records changes and observations in electronic medical record system.

The Skills You’ll Need :

High school education preferred, or up to six months related experience and / or training.

Must have First Aid Certification .

Basic computer skills with experience using electronic medical records / charting required.

Must be willing and able to walk for multiple hours per shift with ease.
