Come join a collaborative team of Social Worker Care Managers and RN Care Managers in our Care Management department, a department that works together collaboratively to provide patient care excellence! The Social Worker creates optimal outcomes for the patient and family by managing complex psychosocial and economic co-morbidities. Through advanced practice skills mobilized resources to reduce risk and serves as ambassador between hospital and community. Enforces the proper application of hospital policies and procedures regarding appropriateness and timeliness of clinical care given.
1. Education: A Master’s Degree in Social Work ( MSW), from an accredited institution and current licensure (LMSW, LCSW) or certification required by state for the specific field.
2. Experience: One year experience of social work in a hospital or community-based setting preferred, including experience providing individual, couple and family support. Experience in Discharge planning.
3. Training/Certification: PRI Screen preferred, CCM certification preferred.
4. Licensure: Licensed Master Social Work (LMSW, LCSW) or certification required by state for the specific field.
5. Specific knowledge, skills and abilities: The ability to work collaboratively with an already established team environment. Health care law and regulations related to acute care and the immediate post-acute continuum, proficient computer skills, psychosocial assessment, risk factor analysis and treatment planning, group dynamics, Crisis conflict and stress management.
