What are the responsibilities and job description for the LPN - Charge Nurse 7pm- 7am position at Montereau?
Essential Functions/Duties/Responsibilities:
- Regularly assess and chart the resident’s condition, monitoring for any significant change and generate appropriate internal and/or external response.
- Administer prescribed medications and treatments to assure the highest level of care for the resident and document all care appropriately.
- Lead and manage direct reports to assure the desired level of resident service, care, and satisfaction. Monitor and evaluate team member performance; provide coaching, training, and corrective actions as required. Assure equitable administration of policies, and compliance with regulatory and legal requirements.
- Communicate medical data to physicians regarding resident’s condition. Collaborate with medical team, note physician’s orders, and coordinate with appropriate pharmacy and/or ancillary department team members to assure follow through with orders for testing, medication, or treatment as ordered.
- Document and complete monthly resident reports, transfer records, and weekly skin reports per Montereau policy and procedure, to assure a high quality of care is maintained and service documentation is maintained for billing accuracy.
- Assist Certified Nurse Aides (CNAs) with personal care for the residents
- Prepare shift change reports and participate in walking rounds with oncoming shift and provide a daily briefing to assure a clear transition between shifts
- Educate residents, residents’ families, staff, and students, when applicable, on medical procedures to assure maximum effectiveness and continuity of care during a resident’s time with Montereau and following discharge.
- Immediately report any incident of resident abuse or neglect, or suspected resident abuse or neglect, to appropriate health services leadership.
