SUMMARY
Assists the Medical Records Director in collecting, and maintaining resident / patient medical records, ensuring accuracy of data, retrieving medical information upon request, maintaining electronic health records (EHR), retrieving medical information upon request, assist in maintaining record keeping procedures and storage of all clinical records in a manner consistent with facility policies and procedures, professional standards and State
and Federal laws and regulations for long-term care (SNF) facilities.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Retrieves patient records upon request from Health Care providers and authorized individuals
Assist the Medical Record Director with audits of the clinical and electronic health records.
Assists the Medical Record Director in monitoring the physician visit schedule.
Assist the Medical Record Director in maintaining, and distributes medical records forms.
Maintain physical and electronic patient files according to established protocols.
Prepare medical records for release to authorized parties, following HIPAA guidelines.
Respond to requests for medical information from patients, insurance companies, and other healthcare providers.
Maintain patient registration information and demographic data.
Maintain confidentiality of patient information and guard against loss or destruction of data in accordance with the facility’s policy and procedures.
Assists the Medical Record Director in maintaining records and completes required forms and documents in accordance with facility policies and state and / or federal regulations.
Prepares charts for new admissions.
Collects resident / patient medical records and scans and electronically files resident / patient documents by uploading them into Matrix under the corresponding resident / patient file while ensuring proper organization and accessibility.
Other assigned duties and responsibilities.
ENTRY QUALIFICATIONS.
High school diploma or equivalent.
Working knowledge of medical terminology.
Proficient in the use of personal computer.

Working in normal office environment, employee may be required to sit, bend, stoop, use keyboard, see, talk and hear. May occasionally lift objects of 10 to 25 lbs. OTHER REQUIREMENTS
Participates in company-sponsored training to safeguard against improper use and disclosure of resident / patients protected health information. Monitors and helps to oversee facility efforts to comply with company policies, procedures and State and Federal law. WAGE RATE FROM $20.00 TO $22.00 AN HOUR DEPENDING ON EXPERIENCE.
