Montgomery General Hospital – Full-Time with Benefits
Montgomery General Hospital is seeking a Community Health Worker to assist high-risk patients with care coordination in their home and community. This role is a vital part of the care team, working under the direction of a physician or mid-level provider and a nurse.
Key Responsibilities:
- Conduct weekly home visits with patients to support care coordination.
- Facilitate communication between patients and their healthcare providers.
- Connect patients with appropriate community resources and services.
- Provide counseling and health promotion as needed.
Requirements:
- No prior patient care or nursing experience required.
- Must be willing to travel within the MGH service area.
- Must provide their own vehicle (mileage reimbursement available).
- Comfortable conducting home visits independently.
Preferred Qualities:
- Trustworthy and reliable.
- Strong interpersonal and communication skills.
- Ability to express empathy and provide support.
- Basic reading and writing skills.
- Knowledge of community resources.
