Position Summary:
Works in accordance with the patient’s Plan of Care and supervised by the Registered Nurse, provides personal care for patients, and assists with their activities of daily living.
Duties and Responsibilities:
Follows the hospice aide plan of care developed for each patient, under the supervision of the Hospice Nurse.
Observes and reports any changes in patient’s condition immediately to Hospice Nurse.
Completes documentation for each visit as directed.
Continually assesses own skill and knowledgebase and actively seeks additional training through available resources.
Minimum Qualifications:
Current certification from the State of Maryland as Nursing Assistant required; certification as Hospice and Palliative Nursing Assistant preferred.
Two years of practical experience in a hospital, nursing home or home care program. Home care or hospice experience preferred. Must have been providing similar services within the past two years.
Communication skills: ability to read, write, and verbally report clinical information to patients, caregivers, and hospice staff. Requires ability to assimilate easily into emotionally charged environment. Demonstrates gentleness and empathy toward patients. Must be able to demonstrate proficiency in required skills.
Will need to lift, position and transfer patients safely. Manual dexterity and mobility needed, particularly in emergency situations. Must be able to follow written and verbal directions. Job can be emotionally stressful.
