Overview: Monument Physical Therapy LLC is a locally owned, outpatient orthopedic physical therapy clinic in Grand Junction, CO. We use an evidence-based and educational approach, in conjunction with manual therapy techniques, to empower patients to get back to their adventure - whatever that may be. Our Physical Therapists spend the entire appointment 1:1 with the patient performing treatments and techniques they cannot do on their own, making the most of the patient's time at the clinic.
We are looking for a Physical Therapist to join our team and provide the highest quality care to our patients. The ideal candidate is passionate about educating patients so they can achieve their goals, motivated to learn and grow professionally, and excited about becoming part of a small, collaborative, and agile team.
Start Date: Early/mid 2025
Duties:
- Evaluate the patient's condition, determine and implement a comprehensive treatment plan using an evidence-based approach, consistently assess the effectiveness of the plan of care, and adjust and modify as needed to achieve the patient’s desired outcomes and goals.
- Complete quality documentation, including progress notes, daily notes, discharge notes, and insurance forms, with proper billing and coding, in a timely manner in accordance with current professional and regulatory guidelines and standards.
- Operate as a team player with the Monument Physical Therapy LLC team to provide a welcoming, inclusive, and positive experience for all patients, as well as fellow team members.
- Work collaboratively with fellow Physical Therapists to learn, expand your skill set, and share your expertise to provide the highest quality care to patients.
- Coordinate with Physical Therapy Aides to provide efficient and effective treatment sessions.
- Achieve performance standards and goals.

Ideal candidate would also possess the following skills and experience:
- Outpatient orthopedic experience.
- Strong manual therapy background.
- Trigger point dry needling experience.
Requirements:
- Master’s or Doctorate of Physical Therapy from an accredited program.
- Current unrestricted license as a Physical Therapist in the State of Colorado, or will have a valid license by start date.
