Moonlight Home Health Care, Inc. is seeking Home Health Care a qualified, Registered Nurse to head our team of RNs, assessing patient health needs, developing personalized care plans, administering medications, managing wounds, educating aides, patients, and their families on managing their conditions, and collaborating with the home health provider to ensure optimal patient outcomes. The successful candidate will have excellent interpersonal skills, working with a diverse, multidisciplinary team to effectively coordinate patient care must be willing to travel as we service clients in Trumbull, Mahoning, Columbiana, Portage, Stark, and Summit Counties.
Responsibilities
- Conduct patient assessments and develop individualized care plans, adjusting care plans as needed
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Maintain accurate and detailed clinical documentation of patient visits, including assessments, interventions, and progress notes
- Educate aides, patients, and their families on health management and disease prevention and detect early symptoms that could lead to a hospital visit
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Communicate regularly with office staff, physicians, case managers, and other healthcare providers regarding patient progress and care coordination.
- Assess the patient's home environment for potential safety hazards and provide recommendations for modifications
- Any other in-office responsibilities as assigned
Qualifications
- Current Registered Nurse license (Ohio)
- 2 years of experience in Director of Nursing position
- Proficiency in patient assessment, documentation, and care planning
- Work collaboratively a diverse, multidisciplinary office staff
- Work independently and make sound clinical decisions in a home setting
- Strong clinical skills in various medical specialties, including wound care, medication administration, and disease management
- Time management skills to effectively manage a caseload of patients across different locations
- Proficiency in electronic health records (EHR) system
