Moonlight Home Health Care, Inc. is seeking an Intake Coordinator to join our team as the initial contact point for the client or referral source to promote the continuity of care. The successful candidate will be responsible for answering the referral source, patient data collection and documentation, and patient referral coordination with case management and nursing staff, while working in-office with a diverse, multidisciplinary team.
Responsibilities
- Manage the intake process for new patients, welcoming new patients, clients, or families and assessing their needs and eligibility for services, ensuring accuracy, completeness, and compliance with regulatory requirements.
- Collect essential information, including medical history, personal details, and insurance information, ensuring accurate and complete records.
- Maintain electronic and paper medical records, ensuring confidentiality, accuracy, and accessibility, and retrieving patient records as needed for appointments, referrals, and medical inquiries.
- Manage incoming calls and emails, scheduling appointments, and providing information and assistance to patients, caregivers, and healthcare providers in a professional and courteous manner
- Filing, organizing, archiving, shredding, and other general office tasks as assigned
Qualifications & Skills
- High School Diploma or equivalent
- Minimum 2 years of customer service or other healthcare related experience; preference working in a home health environment
- Excellent interpersonal skills to interact effectively with patients, families, and other healthcare professionals
- Effective time management skills and strong attention to detail, managing multiple tasks and prioritizing responsibilities
- Efficient organization skills, maintaining accurate records and ensuring all necessary information is gathered.
- Familiarity with electronic health records (EHR) and other relevant software.
- Familiarity with ICD-10 coding guidelines and medical coding principles.
- Ability to work collaboratively within a team environment while maintaining individual accountability.
