What are the responsibilities and job description for the Service Coordinator position at Moore Center Services, Inc.?
The Moore Center has an exciting opening for a full time Case Manager to coordinate client services in the greater Manchester area. This includes evaluating client needs, prioritizing the level of service coordination, determining appropriate and cost-effective services, tracking expenses and documenting outcomes.
Based in Manchester, New Hampshire, the Moore Center is a state and regional innovator with more than 50 years of experience in helping people become productive, fulfilled members of the community. Working closely with individuals, The Moore Center identifies unique needs and creates individualized programs to enhance that person’s quality of life and that of the community as a whole.
Responsibilities include:
Demonstrate knowledge of NH Regulations (He-M 503, 507, 521, 522, 525, 1001, 1201) and ensure compliance with regulations
Develop a Service Agreement using person-centered planning, including goals, objectives, methodology and timelines based on assessments
Continuously review services, assessing appropriateness and progress toward goals and/or treatment
Coordinate RFP process to create a new or different service provider as necessary
Assist individuals in communicating their needs, desires, and preferences, as necessary
Negotiate service contracts with provider agencies that demonstrate quality services
Demonstrate an understanding of budgets
Research community organizations to reduce costs and increase client supports

Qualifications:
Bachelor’s degree preferred; consideration may be given to candidates with an Associate’s degree and a minimum of 2 years of experience working in human services
1 – 2 years of experience working in human services preferred
Proficiency in various software applications, including Word, Excel, and Access
