This position is a Full Time position Memorial Nursing and Rehab Center, our long term care facility.
SUMMARY: Develops and coordinates any and all activities required for and essential to the development of the individual resident’s Care plan, including coordination and facilitation of regularly scheduled interdisciplinary Care Plan conferences. Additionally, responsible for other tasks as assigned by the facility Director of Nurses. May perform duties as a charge and/or floor nurse if conditions warrant.
EDUCATIONAL/EXPERIENCE REQUIREMENTS: Graduate of an accredited school of Vocational or Professional Nursing.
EXPERIENCE REQUIREMENTS: Related experience at a level necessary to accomplish the requirements of the position, minimum of one (1) year of long-term care experience.
LICENSURE / CERTIFICATION REQUIREMENTS: Current Texas LVN or RN license
Current CPR certification
OTHER REQUIREMENTS: Awareness of JCAHO accreditation and DADS licensing and regulations processes. Must be familiar with standard business office computers usage and software.
