Job Responsibilities of a Social Worker at Moravian Manor Include:
- Provide Social Services support for residents entering Moravian Manor Communities in Residential Living and Personal Care.
- Work as an advocate for residents with the intent of helping them maintain their highest level of independence.
- Assist with completion of initial Social Service paperwork, including social histories, discharge plans, needs assessments, and identification of supportive services and care transitions.
- Provide assistance with Medical Assistance process for services.
- Completes needs assessment interviews, enters information into records, and updates need for supportive services.
Job Requirements of a Social Worker at Moravian Manor:- Must have a BS in Social Work from a school approved by the Council in Social Work Education or a BS in a human services field such as Sociology or Psychology.
- Knowledge and understanding of aging population.
- 3-5 years' experience working with seniors
