Position Summary:
We are seeking a compassionate, organized, and detail-oriented Care Coordinator to join our team. This role acts as a critical liaison between home care providers, home health agencies, and insurance companies to ensure that clients receive high-quality, coordinated, and cost-effective care. The Care Coordinator will advocate for clients and help navigate the complex healthcare system, ensuring services are delivered seamlessly and in alignment with individualized care plans.
Key Responsibilities:

Serve as the central point of contact for clients, families, care providers, and insurance representatives.

Coordinate and monitor the delivery of home care and home health services, ensuring continuity and quality of care.

Collaborate with interdisciplinary teams (nurses, social workers, therapists, etc.) to develop and update care plans based on client needs and goals.

Review insurance authorizations, verify eligibility and coverage, and help resolve billing or service-related issues.

Ensure documentation and communication meet compliance, regulatory, and organizational standards.

Track client progress, assess satisfaction, and facilitate adjustments in services as needed.

Educate clients and families on available services, resources, and best practices for care at home.

Maintain accurate records in care management systems and contribute to ongoing quality improvement initiatives.
Qualifications:

Bachelor's degree in Health Administration, or a related field (or equivalent experience).

2 years of experience in care coordination, home care/home health, or case management.

Knowledge of insurance processes, including Medicaid/Medicare and managed care organizations.

Excellent communication, problem-solving, and organizational skills.

Ability to work independently and collaboratively across teams.

Proficient in electronic health records (EHR) and Microsoft Office Suite.

A valid RN or LSW license is a plus
