Responsibilities:

Qualifications:

Education:
- Bachelor’s degree in nursing, public health, or a related field.

Experience:
- Minimum of 2 years as a Registered Nurse in a home health setting.
- Proven experience in a clinical setting, preferably in care coordination or management.

- Current nursing license (if applicable) or relevant certification in healthcare coordination.
