Community Health Connector

Location: Remote (Full-Time)

Compensation: $22/hour
Work Hours: 8:30 AM – 5:00 PM

** Must live in NY as you may have to occasionally drive locally: Bronx-Queens

Position Overview:
The Community Health Connector improves the health outcomes of members with complex healthcare needs by providing support and guidance to access necessary healthcare and social resources. This fully remote role requires bilingual fluency in Spanish and Bengali and involves making a meaningful impact on diverse, underserved populations.

Responsibilities:

• Proactively contact members with health conditions to provide education, support, and assistance navigating healthcare services.
• Advocate for members by helping them utilize health plan benefits, including scheduling appointments, arranging transportation, and addressing missed appointments.
• Connect members to community resources for housing, food, employment, and other social services.
• Assist members in maintaining Medicaid eligibility and accessing financial resources when needed.
• Conduct research to locate disconnected members using internal databases or external resources and reach out through telephonic or electronic communication.
• Use bilingual skills in Spanish and Bengali to build trust and communicate effectively with members from diverse backgrounds.

Qualifications:

• High school diploma, GED, Associate’s degree, or Bachelor’s degree.
• At least 1 year of experience working with underserved or special needs populations.
• Proficiency with technology for remote communication (e.g., video calls, electronic health records).
• Fluency in both Spanish and Bengali.
• Strong communication, organizational, and interpersonal skills with the ability to work independently.
