- Resident Evaluation & Treatment Planning: Assess residents based on their medical diagnoses and create individualized treatment plans to support functional independence.
- Implementation of Treatment Plans: Execute therapy plans designed to achieve residents’ goals, tracking their progress with detailed documentation.
- Progress Monitoring & Reporting: Prepare recertifications, progress reports, and daily notes to ensure consistent resident care.
- Confidentiality: Safeguard resident information by adhering to confidentiality protocols.
- Documentation & Compliance: Maintain accurate records of resident care services and departmental activities.
- Supervision: Provide clinical supervision to occupational therapy assistants as required.
- Care Plan Participation: Actively engage in care plan meetings and ensure comprehensive documentation.
- Departmental Contributions: Assist the Rehab Director with OT-related reports, data tracking, and departmental projects as needed.
Qualifications
- Graduate of an accredited Occupational Therapy Program.
- Current Occupational Therapist license in New York State.
- Experience in the field is a plus, though new graduates are welcome to apply.
