What You Will Do
As a hospice case manager, you will provide and coordinate hospice nursing and palliative care to terminally ill patients, traveling to and providing care in their homes. You will collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of care and support to achieve the highest quality of compassionate care for the patient and their family.
- Assess to determine hospice needs for patients
- Collaborates with the family / caregiver and IDG to meet the emotional needs of the patient and family
- Utilize a combination of agency resources and standards of practice to achieve pain and symptom management for the highest quality of end-of-life care
- Coordinate the total Plan of Care and maintains continuity of patient care by collaborating with appropriate staff and the physician
- Actively participates in Interdisciplinary Team (IDT) conferences
- Provides supervision of aides
Some on Call Hours
Qualifications Required
Current and unencumbered license to practice as a Registered Nurse in the state of PA
Minimum of one (1) year of experience as a registered nurse
Current CPR certification
Valid Driver’s License and reliable transportation
