Rehab Therapy Team Leader

Summary

We are seeking a dynamic individual to join our transitional care rehab team to direct and coordinate our therapy department; assumes responsibility for continuity, quality and safety of services in compliance with state and federal regulations, as well as fiscal intermediaries.

Responsibilities

1. Directs and coordinates therapy department; assumes responsibility for continuity, quality and safety of services in compliance with state and federal regulations, as well as fiscal intermediaries.

2. Participates in activities relevant to professional services, including assignment of appropriate therapy staff and coordination with appropriate health personnel; including daily schedules and staffing to get optimal patient outcomes.

3. Attend necessary community meetings as designated by Therapy Outcome Manager. (monthly QA, daily stand up meetings)

4. Supervises and provides direction to other therapists to ensure quality, compliance with the plan of care, assessment and reassessment of patient’s needs.

5. Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.

6. Participates in the QA/PI program and assures appropriate corrective measures are performed.

7. Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members.

8. Demonstrate commitment, professional growth and competency.

9. Collaborate with physician and nursing staff for plan of care as indicated. (ensures timely assessment, evaluation and PDPM management for skilled patients)

10. Timely and accurately record treatment effects in accordance with established procedures. (ensure timely submission of managed care updates and documentation by team in EMR)

11. Maintain records pertinent to patient treatments; submit documentation timely. (maintain integrity and accuracy in EMR)

12. Maintain compliance with all MORNINGSIDE MINISTRIES/Morningside Ministries policies, procedures and requirements. Maintain compliance with all federal and state laws. Maintain compliance with all agency regulatory guidelines and requirements.

13. Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.

14. Perform general office tasks as required.

15. Perform special assignments, projects, and other duties as required.

16. Responsible for assuring patient/resident safety.

17. Demonstrate commitment, professional growth and competency.

18. Demonstrate the ability to be flexible, organized and function under stressful situations.

Requirements

1. Education: Licensed as at therapy professional in the State of Texas.

2. Training and Experience: Two years’ experience in SNF preferred. Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR. Knowledge of durable medical equipment.

3. Prior management experience preferred.

4. Certifications/Licenses: Current Texas State License as a therapist or therapist assistant, current Texas Driver’s License

5. Employment Variables: Reliable transportation and valid auto liability insurance at company standards to travel between facilities when required.
