What are the responsibilities and job description for the Case Worker - Conservatorship position at MORONGO BAND OF MISSION INDIANS INC?
Under the direction of the Morongo Tribal Social Services (MTSS) and in close collaboration with the Morongo Tribal Court and Legal Departments, and Contracted Fiduciary, provide ongoing case management of the personal affairs of individuals determined by Tribal Court to be incapacitated.
Assess for Vulnerable Adult/Conservatorship care need. Manage Conservatorship of the Person (COP) cases, Investigate and intervene to ensure APS accuracy and compliance. Coordinate activities and partnerships between MTSS, legal department, courts, fire department, reservation patrol and other Morongo and outside resources.
Under the COP (Conservator of the Person) role, manage the care of the person, make decisions on behalf of client’s best interests, and make arrangements to meet the Conservatee's needs in all areas: personal and health care, meals, clothing, housekeeping, transportation, social activities, recreation and other future goals.
Maintain excellent partnership with the Fiduciary firm serving in the COE (Conservator of the Estate) role, communicating financial needs and to protect the Conservatee's income and assets.
Manage court calendars, attend court hearings as needed. Prepare accurate and timely Court reports. Coordinate case filings, activities and consultations between the Conservatee and the family if available. Coordinate expert witness activities when needed.
Assist in crisis interventions including voluntary and involuntary hospitalization and placements. Help conduct home studies when appropriate. Engage, coordinate and monitor services including communication with outside providers, site visits, transportation, appointments, completing forms, assessing and securing placements and services. Maintain case files and contact notes. Update case status and provide reports as requested.
Participate in “On-Call” rotation duties at the Department, respond to hotline inquiries and emergencies, coordinate responses with Administrator and MTSS staff. Participate in staff meetings, trainings, conferences and other meetings as appropriate/assigned.
