What are the responsibilities and job description for the CARE COORDINATOR position at Morris Hospital?
- DESCRIPTION OF POSITION
- The Accountable Care Organization (ACO) nurse coordinates team based care to provide, develop, implement, and manage programs set by Centers for Medicare & Medicaid Services (CMS) and our ACO consortium. These services deliver evidence-based practice that are provided to individuals, families, and/or their communities through effective partnerships with patients, their caregivers, and their physician to achieve high quality care that is patient/family centered.
- QUALIFICATIONS
- Professional/Educational
- BSN required, MSN or APN preferred.
- 3-5 years' experience in a clinical or community health setting.
- Previous Care Coordination and/or Case Management experience preferred.
- Previous experience with health IT systems and data reports preferred.
- Previous experience with mobilizing community resources, navigating through the healthcare continuum and working with disparate populations preferred.
- Ability to speak a relevant second language preferred.
- DCFS Mandated Reporter Certificate of Completion required.

- Proficient in communication technologies (e-mail, cell phone, etc).
