Children's Case Manager
We are seeking an enthusiastic and experienced Case Manager to join our dynamic, multidisciplinary team in Brewer, Maine. Case Managers develop individualized support plans for individuals served, using a person-centered planning process; analyze support strategies and assure that services received by clients adequately meet their needs; and assist individuals and their families to identify strategies that will enable them to meet their goals.
This position works with children who have an intellectual and/or developmental disability who are as young as 6 months old through 20 years old. The Case Manager is responsible for documentation in accordance with regulatory agencies such as the Department of Health and Human Services. The Case Manager represents the Morrison Center in the community and acts as a liaison with other agencies. The Case Manager will perform monthly home visits, school observation, and support families at IEP meetings, team meetings, and medical appointments. Case Managers will ensure that services do not expire and will complete 90 day continued stay reviews with the family.
Education and Requirements
- Bachelor’s Degree
- Experience working with individuals with intellectual/developmental disabilities is preferred.
