Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registern Nurse (RN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including long-term care facilities, hospice care, and infectious disease units. This role requires a strong understanding of medical documentation and patient care systems, as well as the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Responsible for planning, assessing, intervening, co-coordinating and providing direct patient care by utilizing critical thinking skills.
- Communicates with physicians, ancillary caregivers, patients and families regarding the patient’s plan of care.
- Communicates with patients on an age-appropriate level.
- Strong organization and leadership skills.
- Recognizes and meets the needs of special needs patients, physicians, visitors, and facility staff.
- Performs assessments including physical, psychosocial, nutritional, functional, safety, spiritual, cultural, and educational needs.
- Completes supporting documentation for supplies, medications, labs, charges, and equipment used.
- Contacts insurance companies to verify patients’ insurance eligibility, benefits and requirements.
- Requests, tracks and obtain prior authorizations from insurance carriers within time allotted for medications, testing, etc.
- Documents in the clinic record (Chief Complaint, vitals, past medical history, medications, pharmacy, allergies, height, weight, smoking status, cessation offered if a smoker, fall risks, depression screenings and any other quality measures that are required by our organization
- Demonstrates competency in skills verified by Skills Check List.
- Demonstrates patient and associate safety issues by regular and proper use of certain equipment that assists in patient care (Data scopes, EKG machine, wheelchairs, etc.)
- Follows all MCH policies and procedures.
- Operates as a team with other Clinical Nursing staff with the potential to float intermittently to help other departments due to high demand of coverage needed in other clinic areas.
- Reviews various test results and takes appropriate actions.
- Makes nursing decisions regarding patient status and immediate care needs.
- Accurately reconciles and administers all medications.
- Provides patient and family education and makes referrals to other internal/external providers/agencies.
- Demonstrates compassion, empathy, and respect to all customers.
- Coordinates and directs the activities of clinical and non-clinical associates in providing care.
Experience
Candidates should possess the following skills and experience:
- Valid LPN or RN license in Illinois.
- Previous Nursing experience preferred.
- Knowledge of medical documentation practices and healthcare systems.
- Familiarity with infectious disease protocols and patient management.
- Strong communication skills and ability to work effectively within a team.
- Previous experience in a clinical setting is an advantage.
