Job Summary

The Navigation Nurse coordinates the care of patients undergoing surgery across multiple surgical programs and serves as the main point of contact for the patient, their family, and care providers. The nurse helps with patient screenings, assessments, pre-operative preparation. They collaborate with the medical team, ensure timely and effective communication, and assist with patient education.

Essential Functions

1. Serves as a single point of contact for providers of care, patient, and family.
2. Develops and maintain clinical protocols in collaboration with sponsoring physician.
3. Manages any deviation from designed care pathway, coordinating with surgeon and primary care physician.
4. Coordinates with preadmission testing/PAT and ANRP/Anesthesia/Primary Care Physician and/or surgeon to optimize patient’s medical conditions prior to surgery
5. Collaborate with members of the medical team to ensure a continuum of care for the program patient.
6. Works collaboratively and acts as a liaison between the patient, patient’s family, physicians, nursing and ancillary contracted services.
7. Gathers pertinent information about the patient’s medical, psychosocial, and functional status
8. Works within criteria set by the pre-admission/registration processes identified by the anesthesia provider and surgeon, sends appropriate referrals to anesthesia/surgeon for final determination when information warrants.
9. Implements necessary changes in coordination with the medical team to improve patient quality of care and optimization pre-operatively.
10. Identifies barriers to the plan of care and intervenes as appropriate.
11. Keep patient and patient family engaged in the plan of care and moving toward the identified program timeframe for patient outcome goals. Be an advocate for the patient and the program.
12. Works with physicians to develop and maintain clinical protocols.
13. Reviews clinical progress of patients and coordinates communications to referring, consulting and primary care physicians.
14. Uses professional and industry clinical practice guidelines and recommendations to guide patients throughout their care plan and discharge planning.
15. Assists in development of the patient navigator program.
16. Develop patient, community, provider and staff education on the patient navigator program.
17. Works in a collaborative, team-oriented style.
18. Trains facility professional and clinic staff on current principals and standards of care related to patient and family centered care.
19. Maintains confidentiality, demonstrates compassion, empathy, and respect to all customers.
20. Demonstrates and actively promotes an understanding and commitment to the mission of Morrison Community Hospital.
21. Maintains a working knowledge of applicable Federal, State and Local laws and regulations.

Qualifications

1. Active registration for Registered Nurse - Board of Nursing, State of Illinois.
2. Must be BLS and ACLS certified.
3. BSN preferred
4. Previous Patient Navigation experience preferred.
5. Case Management experience preferred.
6. Knowledge of Standard Precautions for exposure to Blood Borne Pathogens.
7. Must possess excellent organizational and communication skills.
8. Must be capable of problem solving, critical thinking, and assessment of patient conditions.
9. Must be able to demonstrate basic computer skills in a Windows environment.
10. Must be comfortable operating in a collaborative, shared leadership environment.
11. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring.

Compensation:

Pay Range $26.00-$35.97 per hour
