About You
- Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) degree from an accredited dental school
- Current, valid dental license to practice in the state or ability to obtain
- DEA certification
- CPR/BLS certification (or ability to obtain)
