Health Coach - Onsite, Part-Time
apree health, the combined entity of Castlight and Vera Whole Health, is dedicated to helping employers and health plans enhance engagement, simplify processes, and establish sustainable cost reductions in healthcare. We are currently seeking a Health Coach to join our exceptional Centralized Clinical Services team.
As a Health Coach, you will play a vital role in supporting diverse patient populations by providing health and wellness coaching. You will collaborate with individuals to co-create personalized action plans aimed at achieving their unique health goals. This position involves facilitating coaching sessions both individually and in groups, which may be conducted virtually or in-person where available. Health Coaching Responsibilities
- Conduct personalized, one-on-one coaching sessions focused on individual health goals, using a member-centered approach.
- apree’s model uses 30 minute visits where coaches collaborate with members to develop actionable plans and SMART goals, providing ongoing support and accountability, adjusting strategies as needed to ensure success.
- Complete thorough documentation of visits, providing a member with a personalized care plan based on coaching sessions.
- Understand members' health conditions and trends supporting them in getting the right level of care.
- Conduct outreach calls to re-engage members and track their participation.
- Ability to correctly perform screening tools and measurement that are appropriate within the Whole Health Coach scope
- Schedule and coordinate appointments, ensuring proper follow-up for optimal health outcomes.
- Facilitate group coaching sessions to enhance engagement and support.
Member Support & Collaboration
- Demonstrate empathy and ownership in resolving member concerns, representing apree health to both internal and external stakeholders.
- Connect members with digital health platforms, health plan benefits, employee programs, and community resources to improve healthcare outcomes helping them overcome barriers to care.
- Play an active role in supporting the care team and in achieving apree health’s goals. Act as a coaching and behavior change expert, offering insights and guidance to the broader care team.
Administrative & Compliance
- Adhere to NBHWC Code of Ethics and health coaching scope of practice.
- Participate in wellness and incentive campaigns and events to drive member engagement and improve outcomes.
- Ensure adherence to company policies, including PHI/PII protection, IT security, and documentation standards. This includes having a private space to conduct coaching calls when working remotely (include if hybrid or remote).
- Escalate clinical or technical concerns to designated teams following protocol.
- Embrace routine feedback from call and case reviews to continuously improve service quality.
- Conduct coaching sessions in a professional healthcare setting, whether delivered in person or virtually, ensuring a consistent and high-quality experience for all patients.
- Perform other duties as assigned, adapting to the evolving needs of the team.
Qualifications:
- Minimum 1-3 years of health coaching experience, preferably in a healthcare setting.
- Bachelor's degree in a health and wellness field preferred
- National Board-Certified Health and Wellness Coach (NBC-HWC) certification preferred, or required within 12 months of hire.
- Strong customer service skills, with a proven track record in healthcare or related industries.
- Solid understanding of the distinctions between coaching, health education, and behavioral health.
- Experience working in telehealth or as part of a remote team preferred.
- Familiarity with EMR systems, Salesforce, Google Suite is an advantage.
- Exceptional verbal and written communication skills, with a professional demeanor.
- Strong organizational skills, with the ability to multitask, manage caseloads, and maintain attention to detail.
- Comfortable in a fast-paced, dynamic environment, with strong change management abilities.
- Flexible to work hours based on company and client needs
- Bilingual or multilingual skills are a plus.
Employee Health Requirements:
- Documentation of Hep B vaccination or proof of immunity (titer).
- Documentation of proof of a negative TB test in the last 90 days before start-date.
- We require CPR certification (BLS) pre-hire and ongoing
- Documentation of annual influenza and COVID-19 vaccinations in compliance with company policy
Physical Demands
- Requires being at a desk for extended periods of time
- Requires being on the phone with members’ insurance companies, providers and others throughout the day.
- Possible travel to client onsite events ask requested
- Manual and finger dexterity and eye-hand coordination
- Requires corrected vision, hearing and speech within normal ranges
Compensation: $22/hr. - $33/hr. & bonus eligible
