Nurse Practitioner
Responsible for managing patient care and treatment (in person, telephonically and/or video).
Primary duties may include, but are not limited to:
- Provides a range of primary, acute and/or specialty healthcare services for patients in clinics, virtually or in home settings. In states that do not require collaborative agreements, functions as the primary care provider.
- Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
- Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices.
- May work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team.
- Formulates and documents care plan. Prescribes medication or other forms of treatment.
- Participates in On-Call program, which may include after-hours, weekend and holiday calls and visitations.
Requirements:
- Requires an MS in Nursing
- Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
- Requires a current, active, valid and unrestricted RN license and NP license in applicable state(s).
- Multi-state licensure is required if this individual is providing services in multiple states.
- For Carelon Health, satisfactory completion of a Tuberculosis test is a requirement for this position.
Compensation: $113,098K - $169,646K/annual salary & bonus eligible
