What are the responsibilities and job description for the Referral Coordinator position at Mosaic Health?
Mosaic Health Utica Medical located on Oneida Street has an immediate opening for a Referral Coordinator to join our integrated care team.
Duties and responsibilities include:
- Coordinating referrals to external health providers and following up to ensure appointment attendance
- Documenting notes and communication in the Electronic Health Record (EHR)
- Maintaining a comprehensive working knowledge of community providers and network services for target population
- Following up with patients who fail to keep appointments and/or are not in compliance with their care plan
- Assisting and empowering patients to be active and engaged participants in their healthcare
Minimum Qualifications:
· High School Diploma or equivalent
· Medical Assistant Certification preferred
· Previous referral experience preferred
· At least one year of experience in a healthcare setting
· Experience with an electronic health record
