Social Work Care Manager

Mosaic Health has an immediate opening for a Social Work Care Manger to join our integrated care team covering multiple site locations in the Mohawk Valley Region (located in Utica and Ilion, NY).

The Mosaic Health Social Work Care Manager aims to improve patient health outcomes, and access to care by assessing and supporting patients to address social determinants of health needs, duties include but are not limited to:

- Attending multi-disciplinary care team meetings to identify patients in need of support with

any of the following needs: social determinants of health, health literacy, patient

engagement, chronic and/or high risk medical, behavioral health or dental needs.

- Providing outreach to patients to assess their social determinant of health needs through any of

the following means: patient support questionnaire, warm handoff, electronic request, or

patient request.

- Maintaining knowledge and understanding of community resources and organizations.

- Obtaining initial New York State of Health Assistor Certification and completes required

mandated training to maintain certification. As a Certified Application Counselor, supporting

Mosaic Health patients and/or community members to apply for health insurance in the

New York State Marketplace.

- Assisting patients to apply for financial assistance programs for which they may eligible,

including Mosaic Health Sliding Fee Discount program, pharmacy assistance, payment plans,

and hardship programs.

- Supporting patients to obtain transportation to healthcare appointments through Medical

Answering Services (MAS) and other contracted transportation vendors.

Minimum Qualification Standards:

• Bachelor’s degree in social work, Psychology, or related field, or 10 years of equivalent

experience

• Case management experience

• Valid NYS Driver’s License as travel is required between other office locations

• Ability to work flexible hours
